Request Authorization of Social Service Agreement with Chicanos Por La Causa to Provide Assessment and Case Management for Guests Under the Age of Sixty at Gateway West

As a result of Request for Proposal RFP-2024-WEHC-SS, the Department of Health, Housing, & Homelessness is entering into a Social Services Agreement with Chicanos Por La Causa to provide assessment and case management for connection to housing or higher level of care services to guests under the age of sixty (60) at Gateway West.
The Department of Health, Housing, & Homelessness has established reimbursement policies. Chicanos Por La Causa will not exceed the total available funds approved in the FY-25 budget for assessment and case management services for guests under the age of sixty (60) at Gateway West.
The Department of Health, Housing, & Homelessness respectfully forwards this request to the Council for consideration and action.
